Azithromycin: Uses, Dosage, Side Effects, and Indications

Azithromycin is a macrolide antibiotic widely prescribed for bacterial infections. It is effective against respiratory infections, skin infections, and sexually transmitted diseases. Its once-daily dosing and short course make it a popular choice for treating infections.

    What is Azithromycin?

Azithromycin works by inhibiting bacterial protein synthesis, preventing the bacteria from growing and replicating. It is commonly used for treating infections caused by a wide range of bacteria that are sensitive to it.

    Common Indications for Azithromycin

    Azithromycin is prescribed for:

    1. Respiratory Infections – Such as pneumonia, bronchitis, and sinusitis.
    2. Skin and Soft Tissue Infections – Including cellulitis and impetigo.
    3. Ear Infections – In both adults and children.
    4. Sexually Transmitted Infections (STIs) – Such as chlamydia and gonorrhoea.
    5. Throat Infections – Including strep throat and tonsillitis.
    6. Typhoid – Dosage and Routes of Administration.

    Typical dosing includes:

    ● For Adults: 500 mg on day one, followed by 250 mg for the next four days (for respiratory infections).
    ● For Children: 10 mg/kg on day one, followed by 5 mg/kg for the next four days.

    Routes of Administration:

    ● Oral Tablets and Suspension: Azithromycin is available as tablets, capsules, and oral suspension.
    ● Intravenous: In cases of severe infections requiring hospitalization.

    Adverse Reactions and Side Effects

While azithromycin is generally well-tolerated, side effects can include:

    1. Gastrointestinal Upset – Nausea, diarrhoea, and abdominal pain are common.
    2. Allergic Reactions – Rash, itching, and anaphylaxis in rare cases.
    3. Heart Rhythm Issues – Azithromycin can cause QT prolongation, leading to heart rhythm abnormalities.

    Contraindications

Azithromycin should not be used in the following situations:

    1. Liver Disease – Patients with liver dysfunction may experience elevated liver enzymes.
    2. Allergy to Macrolides – Those with hypersensitivity to macrolides should avoid using azithromycin.
    3. Severe Heart Conditions – Patients with a history of arrhythmias should be cautious.

Effects on Pregnancy, Liver, and Kidney

    ● Pregnancy: Azithromycin is considered relatively safe during pregnancy (Category B) but should be used only when necessary.

    ● Liver: It can cause liver enzyme elevations; caution is needed for those with preexisting liver issues.

    ● Kidney: No significant dosage adjustments are usually needed in patients with kidney impairment, but monitoring is essential.

    Popular Brand Names

    ● In India: Azithral, Zithrocin.
