    Sildenafil: The Treatment for Erectile Dysfunction and Pulmonary Hypertension

    Sildenafil, a popular medication often associated with the brand name Viagra, is widely known for its role in treating erectile dysfunction (ED). Beyond its common use for ED, Sildenafil is also an effective treatment for pulmonary hypertension. 

    What is Sildenafil?

    Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor that increases blood flow to certain organs of the body.By inhibiting PDE5 (which is responsible for degradation of cGMP) in the smooth muscle of pulmonary vessels it results in the relaxation of vessels and vasodilation, increasing the blood flow and reducing the workload of the heart. In erectile dysfunction, it affects the response to sexual stimulation by causing release of NO resulting in stronger and sustained erections.

    Indications for Sildenafil

    ● Erectile Dysfunction (ED): Sildenafil is most commonly prescribed for ED, helping men achieve and maintain erections.

    ● Pulmonary Hypertension: It is used to increase the blood flow to the pulmonary vasculature.

    Dosage and Administration

    Adult Dosage:

    ● FFor Erectile Dysfunction: 50 mg taken about 30-60 minutes before sexual activity. The dose is adjusted from 25 mg to 100 mg depending on the response and side effects.

    ● For Pulmonary Hypertension: 2.5-10 mg three times a day, intravenous bolus injections. Orally it is given as 5 mg or 20 mg BID depending on the response and severity of the condition.

    Pediatric Dosage:

    ● For Pulmonary Hypertension in Children: Pediatric dosages are determined based on weight and specific conditions and should be guided by a healthcare provider.

    Side Effects of Sildenafil

    ● Common: Headaches, facial flushing, dizziness, upset stomach, nasal congestion.

    ● Serious: Vision changes, hearing loss, priapism (painful erection lasting more than 4 hours).

    ● Pregnancy Category: Category B (no risk in human studies, but should be used cautiously).
