    Amlodipine: Uses, Dosage, Side Effects, and Indications

    Amlodipine is a calcium channel blocker commonly used to treat high blood pressure (hypertension) and chest pain (angina). It helps relax blood vessels, allowing blood to flow more easily, and is a widely prescribed drug for hypertension and  heart-related conditions. Amlodipine is usually taken once a day, and it’s important to take it regularly for the best effect.

    What is Amlodipine?

    Amlodipine works by inhibiting calcium ion movement into the smooth muscle cells of the heart and blood vessels, leading to vasodilation and lower blood pressure. This action makes it effective in reducing the risk of heart attacks and strokes.

    Common Indications for Amlodipine

    Amlodipine is used to treat:

    1. Hypertension – It helps to reduce blood pressure and lower the risk of cardiovascular events.
    2. Chronic Stable Angina – It prevents chest pain due to decreased blood flow to the heart.
    3. Coronary Artery Disease (CAD) – It is used to reduce the workload on the heart and improve blood flow.

    Dosage and Routes of Administration

    Typical dosing for Amlodipine includes:

    ● For Adults: 5-10 mg once daily, depending on the condition and response.

    ● For Children: 2.5-5 mg once daily, as prescribed by the physician.

    Routes of Administration:

    ● Oral Tablets: Amlodipine is taken orally, with or without food at the same time every day.

    ● Oral Suspension (liquid): For those who have difficulty swallowing tablets, amlodipine can also be prepared as a liquid suspension. This form is usually given to children or adults with swallowing difficulties.

    It is not generally available in injectable form

    ● Missed dose: If you forget a dose, take it as soon as you remember, but if it’s nearly time for your next dose, skip the missed one. Don’t take a double dose to make up for the missed one.

    Adverse Reactions and Side Effects

    Though generally well-tolerated, amlodipine may cause:

    1. Swelling (Edema) – Particularly in the legs or ankles is the most common side effect.
    2. Dizziness or Light headedness – Often due to a sudden drop in blood pressure.
    3. Palpitations – Some patients report an increased awareness of heartbeats.
    4. Fatigue – Tiredness is a common side effect.
    5. Gastrointestinal –  Abdomial pain and nausea.
    6. Conjunctivitis eye pain and double vision.
    7. Tinnitus and bleeding from nose.
    8. Muscle cramps.
    9. Breathing difficulty.
    10. Very rarely rashes, gynaecomastia, hepatitis and kidney injury.
    11. Abnormal dreams, anxiety, depression and insomnia.

    Contraindications

    Amlodipine should not be used in patients with:

    1. Severe Aortic Stenosis – This condition can be worsened by vasodilation.
    2. Severe hypotension : Low Blood pressure is a contraindication to amlodipine administration.
    3. Allergy to Amlodipine or Other Calcium Channel Blockers – Hypersensitivity can lead to severe reactions.
    4. Severe Liver Disease – Dosing adjustments are required for patients with impaired liver function.
    5. Breast feeding women

     

    Effects on Pregnancy, Liver, and Kidney

    ● Pregnancy: Amlodipine is generally considered safe in pregnancy but should only be used when clearly needed.

    ● Liver: Patients with liver impairment may need lower doses to avoid potential toxicity.

    ● Kidney: Amlodipine does not typically require dose adjustments in patients with kidney disease, but monitoring is recommended.

    ● There are no known interactions between amlodipine and foods or drinks.
