Topiramate: The Anti-Seizure and Migraine Prophylaxis Drug
Topiramate is an antiepileptic drug used to treat seizures and migraines. It controls both focal and generalized seizures and is often prescribed as part of a comprehensive treatment plan for epilepsy and prophylaxis of migraine headache.
Mechanism of Action
Topiramate enhances GABA (gamma-aminobutyric acid) activity, and antagonizing AMPA/kainate glutamate receptors, inhibiting certain excitatory neurotransmitters, and blocking voltage-gated sodium channels, which helps reduce abnormal electrical activity in the brain that causes seizures.
Indications
● Epilepsy: For both focal and generalized seizures.
● Migraine Prophylaxis: To prevent the frequency and severity of migraines.
● Weight Loss: Off-label use for weight management, in combination with other drugs.
Dosage and Administration
● Epilepsy: Start with 25 mg daily, increasing gradually to 200–400 mg daily, depending on individual response.
● Migraine: Typically 50 mg once or twice daily for prevention.
● Pediatric dosage: Initial dose is 0.5-1 mg/kg/day in two divided doses. It may be titrated with increments of 0.5-1 mg/kg/day at 5-7 day intervals depending on clinical response and tolerance.
Side Effects
● Common: Dizziness, fatigue, weight loss, and tingling in the hands or feet.
● Serious: Kidney stones, glaucoma, and cognitive impairment (difficulty with concentration or memory).
Pregnancy Category: Category D – Use during pregnancy only if clearly needed.
