Zolmitriptan: An Effective Treatment for Migraines
Zolmitriptan is a triptan medication used to treat acute migraine attacks with or without aura (visual disturbances). It works by constricting blood vessels in the brain, which become dilated during a migraine attack, and by inhibiting the release of inflammatory chemicals that contribute to migraine pain. Zolmitriptan is most effective when taken as soon as the migraine starts.
Mechanism of Action
Zolmitriptan works by stimulating serotonin receptors (5-HT1B/1D) in the brain. These receptors cause the constriction of dilated blood vessels and inhibit the release of neurotransmitters like substance P, which contribute to the inflammation and pain experienced during a migraine attack.
Indications
● Acute Migraine Treatment: Zolmitriptan is used to treat acute migraines, with or without aura, in adults.
● Cluster Headaches: It may be used off-label for the treatment of cluster headaches, a type of severe headachethat occurs in cyclical patterns.
Dosage and Administration
● Adults: The starting dose is 2.5 mg taken as a single dose. If the migraine persists or returns, a second dose of 2.5 mg may be taken after 2 hours. The maximum dose per 24 hours is 10 mg.
● Children and Adolescents: Zolmitriptan is generally not recommended for children under 18 years of age for migraine treatment.
Side Effects
● Common Side Effects: Dizziness, drowsiness, dry mouth, fatigue, and nausea.
● Serious Side Effects: Chest pain, irregular heartbeats, severe allergic reactions, and serotonin syndrome(symptoms include confusion, agitation, high body temperature, and muscle rigidity). Ischemic heart disease may be a concern in individuals with cardiovascular risk factors.
Pregnancy Category: Category C – Zolmitriptan should only be used during pregnancy if the benefit outweighs the potential risk to the fetus.
